Guiding your steps
This toolkit supports our 8-week outpatient radiation treatment falls initiative—screen early, recheck at weeks 2 & 6, teach-back in the preferred language, and close the loop in the Electronic Health Record. Quick buttons link to the exact tools we’ll use on shift.
Mission Statement
At our Outpatient Radiation Treatment Clinic in Miami, we equip the entire radiation therapy team—Front Desk, MAs, RTTs, Nurses, and Transporters—with practical tools to prevent falls during and after radiation therapy. Inside you’ll find 3-minute screens (TUG + orthostatic BP), FRID/Beers-guided medication reviews, RT-specific workflow and environment checklists, and ready-to-use team communication tools (SBAR, briefs/huddles/TeamSTEPPS), plus printable patient & caregiver education in English and Spanish—everything you need to screen early, intervene promptly, and close the loop in the EHR without slowing throughput.

Facts about Falls
Falls are a serious risk for adults ≥65—approximately 1 in 4 fall each year, resulting in approximately 3 million ED visits each year (CDC, 2024). The CDC’s STEADI program reframed AGS/BGS recommendations into the three-step approach of Screen, Assess, Intervene (CDC, 2025). The STEADI Algorithm outlines utilization of tools such as the Stay Independent 12-item screen and calculating “risk ≥4,” the 3 key screening questions (unsteady, worry about falling, or falls in the past year), and targeted assessments (e.g., TUG, orthostatic BP, medication review with Beers Criteria, home hazards, vision/footwear), followed by targeted interventions (e.g., exercise/PT, med optimization, home safety, vision care, hydration and compression, education) and 30–90-day follow up (CDC, 2025). STEADI also offers EHR decision support, case studies, provider/ patient/ caregiver education, and continuing education to encourage adoption (CDC, 2025).
Prioritizing safety, one step at a time
Your safety matters—especially during radiation therapy, when fatigue or medicines can make you feel a bit unsteady. Our team is here to keep you safe every step of the way with a quick 3-minute check (walk test + standing blood pressure), a review of any medicines that might cause dizziness, and escorts to and from treatment rooms whenever you need them.

"The Outpatient Radiation Therapy Fall-Prevention Toolkit provided invaluable support and resources during my treatment. I felt safer and more confident thanks to their guidance."
